- comments: observed bulk containers in prep area not labeled. Instructed manager to label bulk containers with common name

- comments: observed a prohibited animals (cat) on site in the facility. Person in charge was made aware that such animals are prohibited from food establishment. Instructed to remove animal from the facility and maintain at all times. Priority foundation #7-40-060. Citation issued

- comments: observed debris around dumpter. Instructed to clean and maintain

60. Previous core violation corrected
- comments: previous core violations not corrected from inspection report# 2572982 issued on 03-22-2023. #47-observed torn gasket in the 2 door cooler in prep area. Instructed to repair or replace and maintain. #49-instructed to clean the following and maintain: interior bottom of fryers to remove grease, exterior surfaces of fryers and grill, underneath grill, bottom of prep table. Clean the interior of refrigeration unit in prep area and reach in freezer where fish is stored. #55-some unnecessary items found in the unused walk-in cooler; instructed to remove to avoid clutter. #55-instructed to clean floors and walls around under and behind cooking equipment, floor in washroom, floor behind refrigeration units and freezers. Clean vent fan on washroom ceiling to remove dust. #56-observed 2 burnt out light bulb in the prep area. Instructed to replace. #58-observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times. Priority foundation 7-42-090. Citation issued
