- comments: observed no procedure and kit on site for responding to vomit and diarrhea. Left one pager and instructed to maintain procedure and kit containing all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.

- comments: observed no soap at exposed handwashing sink located in rear dish area. Instructed to maintain soap at all handwashing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed 9 live roaches in corner of rear prep area near stairs and where loaves of bread area stored. Instructed to remove pests, receive additional pest control service, and maintain facility free of pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed unsealed wood at bar. Instructed to seal and maintain in good condition.

- comments: observed 1 of 2 washbowls in women's toilet room maintaining an improper maximum temperature of 71. 6f. Instructed all washbowl sinks must maintain proper temperatures between 100-120f.
