- comments: noted over 15 live roaches on the floor and walls in the kitchen prep area, dishwashing area, and at the basement dry storage. Instructed to clean and sanitize affected areas, and eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a)

- comments: noted rear exit door not rodent proof with gap at the bottom of door (about 1/4 inch). Instructed to seal gaps to prevent entry points for pests.
39. Contamination prevented during food preparation, storage & display
- comments: food containers are stored on the floor in the dry storage area by the rear exit door. Must provide proper, raised shelving to store food containers 6' off the floor.
43. In-use utensils: properly stored
- comments: noted serving plates on shelves at the kitchen prep area and dishwashing area not properly stored. Instructed to invert all serving plates to prevent dust accumulation.

- comments: noted torn rubber gasket inside walk in freezer door at the kitchen prep. Instructed to detail repair and maintain and/or replace.

- comments: instructed to detail clean the service prep cooler and spills on shelves, countertops of the hot holding containers, all cooking equipment, and around the service coolers with dirt, food particle, and grease. Must also detail clean and maintain the door linings of the service coolers.

- comments: noted outside grease container not maintained with grease on top of container. Instructed to detail clean and maintain grease container and its surrounding. Priority foundation #7-38-020(b).

- comments: noted old food spills on the ceiling at the kitchen prep area, standing water in some areas of the kitchen area, underneath the dishwashing area, and floors along wall base with dirt and food spills. Must clean ceiling and remove the spills, remove pooling water and maintain dry floors.

- comments: noted dirt and grease accumulation inside, on pipes/conduits, and edges of the exhaust hood and filters above cooking equipment by the walk-in freezer. Instructed to detail clean and maintain hood, filters, and pipes/conduits.
