Report date: May 24, 202422. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods inside of the 3 door prep cooler across from the cook's line. Observed eggs at 55f, hamburgers at 52f, cheese at 50f, and sour cream at 51f. Management instructed to maintain all cold tcs foods below 41f. Priority violation 7-38-005. Citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory disclosure on the menu or menu board for raw or undercooked foods. Management instructed to disclose or decribe which foods can be ordered raw or undercooked and link those items to a reminder statement on the menu and menu board. Must add the following statement before the reminder statement:'these foods can be ordered raw or undercooked or may contain raw or undercooked ingredients. ' priority foundation violation 7-38-005.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the 3 door prep cooler with an air temperature of 50f. The cooler was being used to store tcs foods such as eggs and hamburgers. Management instructed to repair and maintain the cooler below 41f. Priority violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed an infestation of insects and rodents on the premises. Approx. 30 live cockoaches were observed on the shelf and on the floor below the hot holding cabinet, around the 3 compartment sink, on the floor of the dining/retail area and in between the top and sides of the countertop at the half wall separating the food prep area from the retail area. Also observed several live insect larvae crawling inside of the bags of grains and pasta on the retail food storage shelves. Also approx. 300 rat droppings were observed on the floor and on storage shelves in the basement and basement storage room. Management instructed to clean and remove all rodent droppings throughout the basement. Additional pest control service is needed to eliminate all insect and rodent activity throughout the premises. Priority violation 7-38-020(a). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed dead cockroaches on the floor throughout the facility. Must clean and remove all dead insects. Observed dead flies inside of the chest freezer in the basement. Management instructed to remove all dead flies.
49. Non-food/food contact surfaces clean
- comments: detail clean all surfaces of all cooking equipment in the food prep area including prep tables, microwave, grill, oven, band saw and slicers, deep fryers, the top of the freezer and hot holding unit. Clean the buildup of grime on the wire storage shelves in the walk-in cooler. Clean the mold growth and buildup on the condenser unit of the walk-in cooler. Clean the dust and debris on the retail food storage shelves.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle provided in the employee washroom. Management instructed to provide.
55. Physical facilities installed, maintained & clean
- comments: remove clutter and unnecessary items stored on the floor in the food prep area, behind the front register, in the front window next to the fruit and vegetable cooler, and in the basement to eliminate pest harborage conditions.

- comments: observed wood sides and ceiling in the walk-in cooler and split wood supports that are not smooth and easily cleanable. Management instructed to cover all walls and ceiling of the walk-in cooler with a smooth and easily cleanable surface.

- comments: seal the gaps in between the top of the countertop and sides of the half wall of the food prep area and dish washing area. Observed a hole in the floor of the basement. Must seal all openings.

- comments: clean the floor throughout the food prep area, under the retail store shelves and basement. Clean the mold growth on the ceiling above the condenser unit in the walk-in cooler. Clean the excessive spiderwebs inside of the doorway to the basement storage room.

- comments: observed no working ventilation fan in the washroom. Management instructed to repair or replace the ventilation fan.
