- comments: 2-101. 11 observed the person in charge without an original city of chicago food service sanitation manager's certificate posted; at the time of the inspection, a copy of the city of chicago food service sanitation manager's certificate was posted. Instructed to have the original city of chicago food service sanitation certificate posted at all times. Priority foundation 7-38-012.
16. Food-contact surfaces: cleaned & sanitized
- comments: 4-303. 11(b) observed that the facility was unable to set up the 2-compartment sink properly to wash, rinse, and sanitize all equipment and utensils. At the time of the inspection, the food prep area is under construction, and the facility does not have access to the food prep area. The facility is using the lunchroom to serve hot foods and wash dishes at a 2-compartment sink. The facility must install a 3-compartment sink to properly wash, rinse, and sanitize. Priority violation 7-38-030(c).
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of live rodent activity on site. Observed about 20 rodent droppings inside of the double door storage closet, about 10 rodent droppings in the cabinet under the 2-compartment sink, about 5 rodent droppings in the corner, all said areas located in the student lunchroom. Observed one rodent droppings in the closet of the pre-k classroom. Instructed to eliminate pest activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation 7-38-020(a).

- comments: 6-202. 15 observed door #5 with a 1/4 inch gap at the bottom across both entrance doors. Instructed to make said door tight fitting and rodent proofed.

- comments: 5-501. 17 observed no covered receptacle inside of the toilet room, immediately next to the lunchroom. Instructed to provide.

- comments: 6-501. 19 observed no self-closing device on the toilet room door that leads into the lunchroom/food prep area. Instructed to provide.

- comments: 6-501. 114 observed crates and boxes being stored directly on the floor in the lunchroom. Instructed everything must be elevated at least 6 inches from the ground.
56. Adequate ventilation & lighting; designated areas used
- comments: 6-304. 11 observed no mechanical ventilation nor a window vent in a toilet room directly next to the lunchroom. Instructed to provide ventilation in the toilet room at all times.
