- comments: observed the rear exit screen door not rodent proof at the bottom. Instructed manager to repair door(tight fitting at the bottom) or keep door closed to prevent entry point for pests.

- comments: observed the wash room door not self closing. Instructed manager to provide and maintain a self closing device.

- comments: floors around the fryer needs cleaning to remove spills(grease).
56. Adequate ventilation & lighting; designated areas used
- comments: observed the filter above the cooking equipment needs cleaning.
57. All food employees have food handler training
- comments: instructed manager to comply with the idph food handler requirements or citations will follow.
