1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation.

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty, upon arrival, while tcs foods are being prepared,handled and served such as (ham, pizza, milk) must be on site at all times. (priority foundation 7-38-012)(citation issued)

22. Proper cold holding temperatures
- comments: observed several tcs foods stored at inadequate temperatures ranging from 46-53 f. Instructed manager to ensure all cold foods are held at 41 f and below. Manager discarded: 1 pound of chichua cheese, 1/2 pound mozzarella cheese, 2 pounds of queso, 1/4 pound goat cheese, 2 pounds turkey, 2 pounds ham, 2 pounds chicken and 3 gallons of milk ($300). Priority violation 7-38-005 citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 3 coolers with inadequate ambient air temperatures ranging from 49-53 f. Instructed manager to ensure all coolers have an ambient air temperature of 41 f and below. Priority violation 7-38-005 citation issued.

38. Insects, rodents, & animals not present
- comments: observed over 50 rodent droppings throughout the facility, including underneath shelving and equipment in the kitchen and in the hot water tank closet. Instructed manager to service all affected areas. Pest control service recommended priority foundation violation 7-38-020(a) citation issued.

- comments: observed 1/4th inch gap in rear exit door. Instructed manager to seal gap.

- comments: observed door of ice machine cover door broken. Instructed manager to repair.

- comments: observed calcium build up on interior of ice machine. Instructed manager to clean.

52. Sewage & waste water properly disposed
- comments: observed inadequate waste water disposal at three compartment sink. When one compartment drains another one floods. Instructed manager to service 3 compartment sink. Priority violation 7-38-030 (c). Citation issued

57. All food employees have food handler training
- comments: observed expired food handler certificates. Instructed manager to ensure all food handler certificates are up to date.
