- comments: observed no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (chicken, beef, steak) has been prepared and served. Instructed a city of chicago certified food manager must be on site at all times foods are being prepared and served. Priority foundation violation 7-38-012. Citation issued
35. Approved thawing methods used
- comments: observed facility not meeting thawing requirements. Food handler thawing frozen chicken on prep table located in the basement. Informed food handler of proper thawing methods ( must utilize running water with enough volatility when thawing). Must meet proper thawing requirements and maintain.
37. Food properly labeled; original container
- comments: observed containers of powders stored on the dry storage shelf with no label. Instructed to provide labels on all unmistakenly powders when taken out of its orginal packaging.
38. Insects, rodents, & animals not present
- comments: observed retail food establishment without prrof of a licensed pest control operator. All retail food establishments shall contract with a licensed illinois exterminator who shall provide insect and vermin services at the establishment at least twice a year. A log book shall be kept for a period of twelve (12) months and include a copy of the service reports and the pest control company�s illinois department of public (idph) health license number. Instructed to provide. Priority foundation violation #7-38-020(c). Citation issued
39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.
41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloths hanging on the side of the hand sink splash guard. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food. Must maintain

- comments: observed a leak at the hot facuet of the three compartment sink. Instructed to repair and maintain.

- comments: observed burnt out lights above the front food prep area and inside the walk-in cooler located in the basement. Instructed to replace and maintain.

- comments: observed no food handler certiciate for employee on site. Instructed to provide and maintain on file.

- comments: observed no city of chicago certified food manager with an allergern certificate on site. Instructed to provide and maintain on file.
