- comments: 5-202. 12 (a)observed improper minimum water temperature of 48. 4f at handwashing sink in grill area. Instructed a handwashing sink shall be equipped to provide water at a temperature of at least 100f through a mixing valve or combination faucet. Priority violation 7-38-030(c) citation issued.

38. Insects, rodents, & animals not present
- comments: 6-202. 15: observed openings at base in between double entrance doors on southside of facility. Observed openings along bases of both garage doors on southside of building(near cars). Observed openings at base of garage door on northside(near dumpsters). Instructed to seal all outer openings to prevent pest entrance

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: 4-904. 13: observed exposed preset knife, fork and tongs on tables in dining area. Instructed manager all preset tableware shall be protected from contamination by being wrapped, covered or inverted.

- comments: 5-205. 15: observed leaking drain pipe of handwashing sink in butcher room. Instructed to repair and maintain.

- comments: 2-102. 13: observed allergen training for one city of chicago certified foodservice manager. Instructed to provide proof of allergen training for all city of chicago certified foodservice managers.
