- comments: observed the person in charge with no city of chicago food manager certificate upon arrival. Instructed manager to ensure certified food manager is on site at all times. Priority foundation violation.
2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on site while tcs foods are prepared and served. Must be on site at all times. The certified food manager arrived on site approximately 30 minutes after the inspection began. Priority foundation violation 7-38-012 citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand washing sink in bar and handwashing sink on service line with no paper towels. Instructed manager to provide manager-provided paper towels. Employee provided paper towels to hand washing sinks. Priority foundation violation 7-38-030(c) citation combined with previous violation.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed food employee using hand washing sink to fill up pitcher. Instructed manager to ensure hand washing sink is used for no purposes other than hand washing. The manager instructed the employee to use the prep sink. Priority foundation violation 7-38-030(c) citation issued.
38. Insects, rodents, & animals not present
- comments: observed 10 small flies in bar area of facility instructed manager to minimize small fly activity. Priority foundation violation 7-38-020(a) citation issued.
52. Sewage & waste water properly disposed
- comments: observed inadequate waste water disposal at three-compartment sink. When one compartment drains, another one floods. Instructed manager to service 3-compartment sink. Priority violation 7-38-030 (c). Citation issued
55. Physical facilities installed, maintained & clean
- comments: observed clutter in the storage area on the upper floor of the facility. Instructed manager to declutter and remove any unnecessary items.
58. Allergen training as required
- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with the completion of training. Must provide.
