A valid city of chicago food service sanitation manager and certificate must be on site at all times while tcs foods are prepared and served to the public. Priority foundation violation. 7-38-012. - comments: observed improper cold holding, found milk and cream at temperatures ranging from 48f - 53f. Instructed to discard, always maintain all cold holding tcs foods at 41f or below at all times. Manager discarded all said tcs foods, cost of all foods $5. Priority violation. 7-38-005. Citation issued.

- comments: found coolers missing thermometers. Must provide.

- comments: found hummus named on self service prepackaged food item. Instructed to provide the common name of the food, a list of ingredients and sub-ingredients in descending order of predominance by weight, including a declaration of artificial colors, artificial flavors and chemical preservatives, if contained in the food; an accurate declaration of the net quantity of contents; the name and place of business of the manufacturer, packer, or distributor; and the name of the food source for each major food allergen contained in the food

- comments: found cases of wine, and multiple freezers stored directly under sewer line. Instructed manager to remove said items or encase sewerline to prevent contamination.

- comments: found employees in food preparation area without hair restraints. All persons shall wear effective hair restraints, such as hairnets, baseball caps, or sun visors, where the hair is covered and contained. Hair spray is not adequate. Mustache or beard restraints shall be used for any facial hair exceeding one (1) inch in length.
