- comments: observed over 50 live small flies throughout establishment. Observed live small fly activity in rear dish washing area and in basement. Must eliminate all pest activity from premises. Priority foundation violation. 7-38-020(a).

- comments: observed exposed waste water line in basement over storage shelves. Must provide suitable contamination for protection.
