Report date: Apr 14, 202228. Toxic substances properly identified, stored, & used
- comments: found personal care items (face mask, vitimins, etc. ) stored throughout the front register area next to single service items. Instructed management to store personal care items in a separate designated area.

- comments: observed milk crates being used as shelving throughout the entire facility. Must remove crates from premises and provide shelving that is free of unnecessary crevices and designed/constructed to be easily cleanable.

- comments: found torn gaskets on the walk-in cooler door. Instructed management to repair broken gaskets and maintain.

- comments: found an accumulation of grease on the cooking equipment in the front prep area. Found a build up of black encrusted food/debris inside the ovens in the rear prep area. Instructed to detail clean said equipment and maintain.

- comments: observed clutter and unnecesary items in the front storage area behind the bread display cases. Must remove clutter, celan, and reorganize area.

- comments: found a heavy build up of old food splatters covering the wall behind the cake decorating area. Found an accumulation of dirt and grime on the floor between the bread proofer and ovens, and the floor inside the walk-in cooler. Instructed management to detail clean said areas and maintain.

- comments: observed cardboard being used as floor mats in the front register area and the rear prep area. Must remove and provide floor coverings that are a non-absorbent, non-corrosive, smooth, easily cleanable surface.

- comments: found multiple food handler certificates expired. Instructed management that all food handling employees must show valid proof of training.
58. Allergen training as required
- comments: found no proof of allergen training on site for all city of chicago food managers. Instructed management that all city of chicago food managers must complete allergen training and keep certificate on site.
60. Previous core violation corrected
- comments: previous core violations from 3/23/2021 report #2497280 not corrected: violation #37 (3-302. 12) must properly label food storage containers when food is not in the original container. Violation #55 (6-201. 11) observed a few water damaged ceiling tiles in rear food preparation area. Must repair replace ceiling tiles. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
