- comments: observed a slight debris accumulation encrusted on the interior of the ice machine. Must remove debris, detail clean inside ice machine/have serviced and maintain ice machine at all times.

- comments: observed 3 live cockroaches underneath the countertop near the 3-compartment sink at bar area. Instructed to detail clean and remove evidence of pest activity and sanitize affected areas;additional pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued

- comments: instructed to properly store and invert kitchenware (forks,knives etc. )to prevent contamination before use (handles-up).

- comments: observed stained cutting board worned-out with deep seams at bar area. Instructed to resurface or replace the cutting boards and maintain.

- comments: observed storage racks in poor repair and with mold-like build up in the walk-in cooler. Instructed to repair,repaint or replace the shelves and maintain.

- comments: observed missing sink stoppers for the dish washing 3 compartment sink at kitchen prep area. Instructed to provide 3 adequate working sink stoppers and to maintain.

- comments: instructed to detail clean the grease on the outside of the deep fryer, prep table next to the grill and the 4 burner stove and maintain.

- comments: observed water leak at drain pipe underneath the kitchen prep/3-compartment sink. Instructed to repair plumbing leak and maintain.

- comments: observed dirty floor at bar with litter debris and broken glass. Instructed to clean floors in detail and maintain.

- comments: instructed to detail clean and organize basement rear dry storage and remove unnecessary items,unused kitchen equipments due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.

- comments: observed dim lighting inside the basement walk-in-cooler. Instructed to provide brighter bulb and maintain adequate lighting.
