- comments: observed three hand washing sinks not accessible for hand washing. Front hand washing sink in pizza prep area, missing drainpipe and covered with linens, hand washing sink in rear prep area backing up and covered with pizza box, hand washing sink in beverage area covered with ice containers. Instructed manager to repair all hand washing sinks and make accessible for proper hand washing. Priority foundation 7-38-030(c) citation issued

- comments: observed chicken sausage 64. 8f and raw sausage 69. 6f stored at improper temperature on prep table. Instructed manager to discard. All cold tcs food must maintain 41f or below during storage, preparation, display and service. Priority 7-38-005 citation issued.

- comments: observed over 60 mice droppings scattered on floor behind door in second floor water heater storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed pizza boxes stored on floor in prep area. Instructed manager to elevate all pizza boxes six inches off floor to protecyt from contamination.
