- comments: still finding evidence of appx. 150 or more rat and mice droppings on top of food storage containers,in meter closet,on ledge behind cooler in bakery prep area,on floor around floor drain,under ice machine,in walk in cooler,rear storage area in between equipment and inside of cages on top of articles,on floor in between wall and lockers in women's toilet room and where needed. Also evidence of 5 live roaches at underside of dish machine and lip of scrape sink area. Must remove droppings. Clean and sanitize areas. Must have a pest control operator to service premises on or after 1/29/2021. (no citation issued)
