1. Person in charge present, demonstrates knowledge, and performs duties
- comments: person in charge does not have a city of chicago sanitation certificate. Priority foundation violation no citation issued.

2. City of chicago food service sanitation certificate
- comments: found no certified food manager on site while tcs foods (chicken, hotdog, pizza etc) are being prepared and served. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present at all times when handling tcs foods. Priority foundation violation 7-38-012 citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at all handwashing sinks at front and back prep areas. Instructed to provide and maintain.

25. Consumer advisory provided for raw/undercooked food
- comments: found incomplete consumer advisory disclosure and reminder statement on the menu. Management instructed to disclose which foods can be ordered 'raw' or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food borne illness. Priority foundation violation 7-38-005

- comments: observed no thermometer in all cold holding units. Instructed to provide and maintain.

- comments: observed two knifes stored in betwen the pipes in the rear food prep area. Instructed to store all cooking equipment properly.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed food debris underneath the prep tables in the rear food prep area. Instructed to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris and dirt on the floor all throughout the rear prep area and in the basement. Instructed to clean and maintain said areas.

57. All food employees have food handler training
- comments: observed no food handlers certificates on site. Instructed to provide and maintain records on site at all times.

58. Allergen training as required
- comments: observed no allergen training on site. Instructed for all certified food managers to obtain an allergen training certificate and maintain records on site at all times.
