- comments: observed no certified food manager present while tcs foods (pasta at 136f) was served. Management instructed that a certified food manager must be present at all times while tcs foods prepared or served. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: no hand soap was provided at the exposed hand washing sink in the food prep area. Management instructed that all hand-washing sinks must be supplied with hand soap at all times. Priority foundation violation 7-38-030(c). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed over 100 mouse droppings located in the following areas: on the floor and storage shelves in the utility room located next to the nw corner exit, middle classroom area along the walls and behind the shelves, hvac closet in the middle classrom, small closet next to main entrance, food storage area next to the south classroom, south classroom closet next to the emergency exit on the se corner of the builing, and storage room with the trap door to the basement. Management instructed to clean and remove all rodent droppings in all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: seal the gaps between the floor and the wall in the food storage area located next to the south classroom.
56. Adequate ventilation & lighting; designated areas used
- comments: the ventilation fan in the north classroom children's washroom is not operational. Must repair or replace the ventilation fan.
