- comments: dry food products stored inside plastic containers must be labeled with common name: beans etc.

- comments: infestation of live raoches on site. We observed 5 live roaches inside the prep cooler at the right door, hinges, and rubber gasket. Over 20 live baby roaches are going in and out from inside a broken rubber gasket from the same cooler. Live over tree adult roaches inside the rubber gasket at the storage cooler in the prep area. Two adult roaches atcrackes of counter behind the bar area. Instructed to contact pest control. Remove, clean, and sanitize all areas: inside prep cooler, etc. Must seal all cracks and crevices. Priority violation:7-38-020(a)citation issued

- comments: outer opening not protected. Front and rear door with 1/4 to 1/2' gap bottom and between doors. Rodent/insect proof said doors, to minimize pest entry

- comments: the condenser unit inside the two-door cooler in the prep area is leaking. We observe ice at the vent cover of the unit. Instructed to repair.

- comments: no quaternary chemical test strips on site to check sanitizing levels for the three-compartment sink that was set with quaternary sanitizing solution. Must provides quaternary test strips. Priority foundation violation 7-38-005 citation issued.

- comments: need to repair hot water handle at handwashing sink in dishroom.

- comments: waste water backs onto the floor in the dishroom when water is released by the three-compartment sink or when the faucet is turned on. Instructed to repair issue. Priority violation 7-38-030(c). Citation issued

- comments: uneven and with grime cement floor between the kitchen and walk-in beer cooler. Repair issue. The floor must be smooth, clean, and non-absorbent material.
