- comments: found no soap at the employee restroom handwashing sink. All handwashing sinks must be supplied with handwashing soap at all times; instructed manager to provide. Priority foundation violation 7-38-030(c). Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed old food debris buildup on food contact surfaces (the meat grip surface and blade) of the deli meat slicer. Instructed management to clean, sanitize, and maintain. Priority foundation violation 7-38-005. Citation issued.
22. Proper cold holding temperatures
- comments: observed tcs foods at improper temperatures inside of the prep cooler. Observed hot dogs at 49. 5f, cut tomatoes at 51. 5 f, sausage patties at 52. 4 f, and cooked chicken at 52. 4 f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: found the prep cooler at an improper cold holding temperature of 51. 5 f during the inspection. Instructed management that prep cooler must maintain a proper cold holding temperature of 41f or below at all times. Prep cooler tagged and held for inspection by the chicago department of public health and may not be used until re-inspected by cdph and maintaining a proper cold holding temperature of 41f or below. Priority violation 7-38-005. Citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: found the 2-door true prep cooler near the walk-in cooler broken and currently not in use. Must repair or remove cooler from premises.
55. Physical facilities installed, maintained & clean
- comments: found old food, duct tape, and dirt inside floor drain in front prep area. Must clean and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: found no lighting in walk-in cooler. Must provide.

- comments: observed no proof of allergen training on site. Must provide proof of allergen training for all chicago certified food managers.
