- comments: observed no verifiable health policy on site at time of inspection. Left template and instructed to maintain health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. No citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure on site for responding to vomit and diarrhea. Left one pager and instructed to maintain procedure and kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. No citation issued.
38. Insects, rodents, & animals not present
- comments: observed 2 live roaches near soap dispenser in kitchen, 1 live roach on box style freezer, and 4 live roaches on wall and electric outlet behind 3 door cooler. Additionally, observed 10 rodent droppings in cabinet underneath dump sink behind dining room and 3 rodent droppings on floor next to dump sink. Instructed to remove droppings and roaches, receive additional pest control service, and maintain facility free of pests. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed leak coming from faucet of 3 compartment sink and floor drain floods floor with water when large amount of water is released from sink. Instructed to repair and maintain in good condition.

- comments: observed heavy build up of grease, grime, dirt, and food debris on interior of fryer and on floor of cook line. Instructed to detail clean and maintain in good condition.
