10. Adequate handwashing sinks properly supplied and accessible
- comments: 6-301. 12 observed no paper towels located in the 3rd floor boys toilet room. Instructed to provide. Employee provided paper towels during the inspection. Priority foundation 7-38-030(c).

10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12(a) observed inadequate hot water at the portable hand washing sink located in the serving cafeteria of the annex school dining room unit. Instructed to provide adequate hot water (at least 100f) at the portable hand washing sink in the mobile cafeteria area. Priority violation 7-38-030(c).

38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed entrance doors #7 and #8 with a 1/4 to 1 inch gap at the bottom of the doors. Instructed to make said doors tight fitting and rodent proofed.

- comments: 6-501. 111 observed evidence of pest activity in the food prep area. Observed about 10 rodent droppings on the floor and stairs in between the west stairwell and the stove, about 15 rodent droppings in between double door oven and hot holding unit, about 10 rodent droppings around the 3 compartment sink grease trap and under the refrigeration units, and about 2 rodent droppings on service tables located under the windows, all said areas located in the food prep area. Observed about 5 rodent droppings in the engine rooms where refrigeration units are located. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation 7-38-020(a).

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 observed one of two hand washing sinks out of order in the boy's 3rd floor toilet room. Instructed to repair.

52. Sewage & waste water properly disposed
- comments: 5-402. 13 observed the toilet rooms located in the annex school unit out of order. Per the engineer, toilet rooms were being rotted due to sewage backup. Instructed to convey sewage at all times properly. Priority violation 7-38-030(c).

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 15 observed the outside garbage dumpster overflowing with garbage and lids not tight fitting on both garbage dumpsters and the recycling dumpster. Instructed never to allow garbage to overflow and always maintain outside dumpster lids closed and tight fitting. Priority foundation 7-38-020(b).

55. Physical facilities installed, maintained & clean
- comments: 6-201. 11 observed holes in the stairwell and walls in various areas of the food prep area, uneven surfaces and multiple holes by the stairwell located by the stove top, and uneven surfaces and holes in the wall under the 3 compartments and one-compartment sinks. Instructed to seal all holes in the walls and provide a smooth, easy to clean surface. Observed lower shelves with debris across the food serving area in the main kitchen. Instructed to clean.
