- comments: must post hand washing signs at all handwashing sinks in prep and toilet room areas. Instructed to maintain.

- comments: must provide a metal stem thermometer onsite to ensure the accuracy of hot or cold food temperatures. Instructed to maintain. Priority foundation 7-38-005 citation issued.

- comments: observed over 200 rodent droppings (mice & rat) throughout the facility; on food storage shelves, floors (corners) and countertops behind front service counter. On floors (corners) in rear storage area, under 3-comp sink, in staff toilet room, under storage shelves on sales floor, and throughout the basement storage area. Instructed to remove droppings, and contact pest control for service, and maintain. Priority foundation 7-38-020(a) citation issued.

- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain.
