- comments: manager provided no proof of employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation. 7. 38. 010. Citation issued.

- comments: observed certified food manager smoking after entering facility. Ashtray with cigaretts' buts on the table in kitchen area.

10. Adequate handwashing sinks properly supplied and accessible
- comments: noted no hot running water at handwashing sink by 3c sink. Instructed to have hot and cold running at all hand wash sinks. Priority violation 7-38-030(c)

- comments: noted food packages in refrigerator and freezer adulterated and not in good condition that protects the integrity of the contents, prevents exposure or is free of potential contaminants. Priority foundation violation #7-38-005. Citation issued.

- comments: observed an open container of sausages stored below an open container filled with water catching dripping water from refrigerator pipes. Instructed to store food according to food code. Priority violation issued 7-38-005

- comments: observed excessive black and slimy substance build up on inside interior panel of two door refrigerator and dripping of the water onto food for human consumption. Must remove build up, clean, and sanitize. (priority foundation 7-38-005)

- comments: observed the following tcs food not meeting the required temperature of 135 f degrees or above: sausages internal temperature at 81. 1 f, stored in an open container and placed at the rack with disposable food boxes. The manager immediately and voluntarily disposed of 2 lbs of products worth $5. 00 through the denaturing process. Priority violation #7-38-005. Citation issued.

- comments: observed that pest control log book is missing required information. Missing record of visits, missing copy of service ticket that sets forth pest control idph license number. Must provides and maintains all required pest control records and documents. Priority foundation violation. 7-38. 020 (c) citation issued.

- comments: noted over 200 mice dropping on shelves in the kitchen, in the food prep area, on the counter at the serving area, and floors throughout the facility. Also, over 5 flying insects are in the kitchen prep area and around the serving counter. Instructed to clean and sanitize affected areas. Recommended to consult with pest control company. Must also eliminate the flying insect problem and pest control upon being re-inspected. Priority foundation #7-38-020(a)

- comments: cutting board with open seams and dark stains. Must refinish or replace damaged or stained cutting boards.

- comments: observed no covered receptacle for proper disposal of sanitary napkins in employee washroom. Instructed to provide covered waste receptacle.

- comments: garbage can in the prep areas are full and uncovered. Instructed to provide lids.

- comments: noted clutter of equipment and other unused items on floors in storage rooms. All items must be elevated to allow cleaning and remove all unnecessary items.

Walls with food dirt and food spills. Cracks on the walls and floors. Instructed to detail clean and sanitize, especially areas with pest activity, and clean and maintain walls with cracks and spills. Must upgrade housekeeping.

- comments: observed exposed ceiling above the food prep area. Instructed food manager to cover the ceiling so that it is sealed, smooth, and easily cleanable in all prep areas.

- comments: observed no food handler's training certification from employee. Instructed that all employee must comply with food handler's training by the food handling regulation enforcement act

- comments: noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.

- comments: observe employee smoking in the kitchen and ashtray with evidence of cigarette and cigarette ashes on the table in the kitchen. Instructed to stop smoking and remove the ashtray. Citation issued. 7-32-010.
