- comments: no certified food manager present with a valid city of chicago food service sanitation certificate while tcs foods (gravy,soups,eggs etc. ) are being prepared and served. Certified manager javaid ahmed arrived on site at 9:05am. Instructed to maintain employee holding city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012,citation issued.

- comments: observed gap opening approx 1/4 inch at bottom left corner of the rear exit door. Must pest proof door all across the bottom to prevent pest entry and maintain.

- comments: observed approximately 15-20 rat droppings along wall behind hot water tank in the rear near rear exit door and on floors in corners and shelves throughout attic. Instructed the person in charge to remove all rodent droppings, clean surfaces, and maintain premises free of pest. Recommended receive additional pest control service. Priority foundation violation 7-38-020(a). Citation issued

- comments: observed excessive build up of encrusted grease deposits along food- and nonfood-contact surfaces of cooking equipment (griddles, stove, surrounding stainless steel walls and supports) throughout the main cook line area. Instructed to clean food- and nonfood-contact surfaces of equipment at adequate frequency and with appropriate cleaning materials to eliminate encrusted grease deposits on cooking equipment and maintain at all times

- comments: observed the 2 compartment sink leaking when its in use. Instructed to reapir and maintain

- comments: observed soiled mop heads not properly stored. Instructed manager to hang mop heads to prevent insect breeding.
