- comments: no reminder and disclosure and asterisk on the menu for all food being serve raw or undercooked on premises. Priority foundation 7-38-005 citation issued.

- comments: all dry bulk food containers must be properly labeled with common name.

38. Insects, rodents, & animals not present
- comments: found evidence of mice dropping in the rear storage room around the water heater, underneath the wire rack shelvings approximately over 35 total. Instructed to remove the droppings,clean and sanitize the affected areas,inform the pest control technician and have additional inspection by pest control technician. Priority foundation 7-38-020(a) citation issued.

- comments: all wiping cloth for wiping the prep tables, chopping board,coolers must be stored on a bucket wih sanitizing solution in between uses.

- comments: ice scoop must be stored on a separate container, handle up to prevent possible contamination.
