- comments: observed approximately 30 live flies on walls and ceiling in front coffee station area and rear dishwashing area. Must clean and sanitized affected areas and whenever needed. Additional pest control service is needed to eliminate flying insect problem. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed dirt and debris on floor underneath cooking equimnet, rear dishwashing area and underneath food storage shelving in 2nd floor storage area. Must clean and maintain.

- comments: observed unnoperable ventilation vents in the unisex washrooms. Must repair and maintan operable mechanical ventilation at all times

- comments: observed previous core violations not corrected from inspection report#2595413 on 06/07/2024. #49. Interior of the cooks line two door cooler with excessive food spillage. Instructed to clean and maintain. #55-ceiling vent above the three crtment sink with excessive dust. Floors surrounding the rear hot water tank with dirt and debris. Instructed to clean and maintain all. #57. Must provide proof of food handler training or certificates for all employees. Priority foundation violation 7-42-090 citation issued.
