2. City of chicago food service sanitation certificate
- comments: found no valid city of chicago food service sanitation manager certificate posted on site while time/temperature control for safety foods (chicken) are being prepared and served to the public. A valid city of chicago food service sanitation manager certificate must be on site at all times while tcs foods are prepared and served to the public. Priority foundation violation. 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand drying provision at any handwashing sinks on site. Noted electric hand dryer inoperable in both women and men's toilet rooms. All handwashing sinks must be equipped with a hand drying provision at all times. Instructed manager to provide. Priority foundation. 7-38-030(c). Citation issued. Hand drying provision added at time of inspection.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing cleanser at handwashing sink in rear food prep area front bar and employee toilet room. All handwashing sinks must be equipped with hand-cleaning liquid at all times. Instructed manager to provide. Priority foundation violation. 7-38-030-(c). Citation consolidated with above violation.

16. Food-contact surfaces: cleaned & sanitized
- comments: found an excessive amount of black mold-like substance on the food contact surface of the ice machine behind the panel. Ice is used for human consumption. Must clean, sanitize and maintain. Priority foundation violation. 7-38-005. Citation issued.

23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety foods (tcs): a variety of pasta sauces held for over 24 hours without discard date. Instructed facility to provide discard date for any ready-to-eat tcs foods held for over 24 hours. Priority foundation violation. 7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: found no thermometers in the reach-in cooler located in the food prep area. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature or a simulated product temperature in the warmest part of a mechanically refrigerated unit and the coolest part of a hot food storage unit. Instructed to provide.

39. Contamination prevented during food preparation, storage & display
- comments: found ceiling debris in the basement on sealed liquor bottles. Instructed to clean, sanitize and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed prep cooler cutting boards in front and rear prep area and food service hotline discolored with deep grooves. Instructed to resurface and maintain or replace.

51. Plumbing installed; proper backflow devices
- comments: found no running when hot water is turned on at utility sink. Instructed to repair and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: found light bulb directly over food prep area burnt out. Instructed manager to replace.

60. Previous core violation corrected
- comments: found continued non- compliance from inspection report # 2561577 conducted on 08/02/2022. The following previous core violations were not corrected: 6-301. 14 observed hand washing signs are needed at all hand washing sinks in the bar and food prep area. Instructed to provide. 3-302. 12 observed dry storage containers without product name labels. Instructed to provide for all items that are out of the original packaging. 6-202. 15 observed front entrance, side entrance, and basement gangway doors not tight fitting and rodent proofed. Instructed to make all said doors tight fitting and rodent proofed. 4-502. 14 observed facility has three large containers of shucked molluscan shells being stored in the basement. Employee states, ' molluscan fish is shucked, made in a puree, and the shells are sanitized and used for a dish' instructed never to use molluscan shells more than once as stated in the food code, plus shells can not be sanitized due to being porous, all food contact surfaces must be smooth and easily cleanable. 5-205. 15 observed low water pressure at the hand washing sink at the bar when hot and cold water is running. Instructed to increase water pressure and maintain. Instructed manager to comply. Priority foundation violation. 7-42-090. Citation issued.
