- comments: must provide hand washing sign at hand sink in women's staff toilet room.

- comments: observed evidence of appx. 100 or more mice droppings on ledge of dish storage rack,on floor along wall base of furnance,equipment storage rack,inside of pan on shelf in same area,on floor next to green cabinet. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 12/8/21. (priority foundation 7-38-020(a)(citation issued)

- comments: must repair or replace worn door gasket on walk in cooler.
