3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Instructed management to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010, no citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops, and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005, no citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed black slimy substances on interior water components of the ice maker, dripping into ice 'ice is used for human consumption. ' management was instructed to turn off the machine, remove ice and have the interior of the ice machine cleaned and sanitized and maintain. Priority foundation violation 7-38-005, citation issued.

38. Insects, rodents, & animals not present
- comments: observed no pest control log book provided from a licensed idph pest control company. Management instructed to provide a pest control logbook with a recent pest control inspection from a licensed idph pest control company. Priority foundation violation 7-38-020(c), no citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed no splash guard at handwashing sink next to the dishwashing 3-compartment. Instructed to provide and maintain.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed missing sink stoppers for the dishwashing 3-compartment sink. Instructed to provide and maintain 3 adequate working sink stoppers.

49. Non-food/food contact surfaces clean
- comments: instructed to detail clean the grease buildup inside of the grill area and interiors of prep coolers with dried-up food debris.

51. Plumbing installed; proper backflow devices
- comments: backflow prevention device not located at coffee expresso system unit. Must install so backflow prevention device may be located to be serviced and maintained.

52. Sewage & waste water properly disposed
- comments: observed the dishwashing 3-compartment sink not maintained; drain pipe wastewater backing up on the floors from the grease trap underneath the mentioned sink. Instructed to repair plumbing issues and detail clean and sanitize areas. Priority violation 7-38-030 (c),citation issued.

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize rear dry storage room and remove unnecessary articles due to potential pest harborage and elevate stock items off the floor at least 6' inches to facilitate cleaning.

- comments: instructed to remove accumulations of dust on ventilation covers on wall above the dishwashing 3-compartment sink area.
