- comments: observed large white round containers of flour not labeled. Instructed all food items not in original container must be labeled with common name of food.

- comments: observed approximately thirty small flies and three large flies in rear dishwashing area, in areas such as surface of large grease container, utility sink, three compartment sink and on large bottles of sauce and seasonings. Instructed to eliminate pest activity, clean and sanitize affected areas. Recommended pest control service facility on or after 9/18/2023. Observed approximately twenty mouse droppings behind three compartment sink, along base of wall underneath shelving units and large white round containers in hallway storage area. Instructed to eliminate pest activity, clean and sanitize affected areas. Recommended pest control service facility on or after 9/18/2023. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed soda crates used for elevation in front prep area. Instructed to obtain adequate shelving to elevate all food and nonfood items at least six inches off of floor for ease of cleanability and to prevent areras for pest harborage. Observed cardboard used as shelf liner in walkin cooler and on prep tables. Instructed to remove cardboard as shelf liner. All nonfood contact surfaces must be smooth and easily cleanable.

- comments: observed red and blue cutting boards stained black and deeply grooved. Instructed to resurface or replace cutting boards and maintain in good condition.

- comments: observed faucet of three compartment sink not long enough to reach third compartment closet to utility sink. Instructed to install faucet long enough to reach all three compartments of three compartment sink. Priority foundation violation 7-38-025 citation issued.

- comments: observed interior of two and three door cold holding unit with food debris and buildup on interior surfaces and gaskets. Instructed to detail clean and maintain. Observed food debris in cold holding display cooler interior. Instructed to detail and maintain.

- comments: observed buildup on interior surfaces of handwashing sink in dishwashing and prep area. Instructed to detail clean and maintain.

- comments: observed bags of trash and boxes stored on ground next to dumpster. Instructed all trash and boxes shall be stored in dumpster so that they are inaccessible to insects and rodents. Priority foundation violation 7-38-020(b) citation issued.

- comments: observed stained, damaged or missing ceiling tiles throughout premises, in areas such as dishwashing, prep area, washroom and waiting area. Instructed to replace ceiling tiles and maintain.

- comments: observed dust on vents of all cold holding units in prep area. Instructed to clean and maintain. Observed dust and grease on filters, interior and exterior of hood. Instructed to clean and maintain. Observed dust on ceiling vent cover in front prep area. Instructed to clean and maintain.
