Report date: Oct 7, 202438. Insects, rodents, & animals not present
- comments: observed 30 mice droppings scatterd in the utility closet, noted 10 mice droppings under crate in the washroom, and 15 mice droppings scatted in the corners in the rear storage/dish washing area. Also note 25 live flies scatterd in the rear storage area, the washroom and dish washing area. Intructed to remove and detail clean all affected areas. Also call pest control for service. Priority foundation citation issued #7-38-020(a).

- comments: observed employee's in the food prep areas without proper hair restraint. Must provide and maintain.
41. Wiping cloths: properly used & stored
- comments: observed wiping clothes laying on counter top. Prep tables, around and in sinks. Must properly store when not in use to prevent futher fly issues.
49. Non-food/food contact surfaces clean
- comments: must detail clean shelves inside the walk in cooler,reach in coolers and freezer. Also clean bottom shelves of prep tables. Must remove spill and debris.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the employee's washroom dirty. Must clean the fixture and floors.

- comments: observed the self closing device on the washroom door in poor repair. Must repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: must detail clean floors through out to remove spills, debris and litter, esp where droppings were found. Wall need detail cleaning in the rear prep and dish washing areas to remove food splatter. Also clean floor drains.
