Instructed management to provide a pest control log book with up to date service reports and all other necessary documents that are needed inside log book such as pest control license, chemicals used, map, etc. And to keep log book on site. - comments: found an accumulation of food and dirt on the floor underneath the oven in the front prep area. Instructed management to said area and maintain.

- comments: found heavy dust and grease accumulation on the ventilation hood located over the oven and heavy dust build up on the ventilation cover inside the restroom. Instructed management to clean said areas and maintain.

Saturday, 11/27/21. Had the cauliflower crust with pepperoni, beef, onions, mushrooms and green peppers.
