10. Adequate handwashing sinks properly supplied and accessible
- comments: observed rear dishwashing hand sink not accessible for use. The sink is blocked by shelving, stored milk crates, and chemicals. Instructed the handwashing sink must be accessible for use at all times. Priority foundation violation 7-38-030(c) citation issued.

- comments: observed no soap in at the rear dishwashing hand sink. Must provide. Priority foundation violation. See the above violation #10 for citation.

16. Food-contact surfaces: cleaned & sanitized
- comments: interior ice machine upper shoot where ice is dispensed with black slime substance noted dripping down into ice. Instructed to clean and sanitize all ice contact areas inside the machine. Priority foundation violation violation 7-38-005 citation issued.

35. Approved thawing methods used
- comments: observed foods being improperly thawed at room temperature on a prep table. Reviewed proper thawing methods.

36. Thermometers provided & accurate
- comments: no internal thermometer inside the rear tall two-door cooler, and the external thermometer is not operational. Must repair or provide an internal thermometer.

38. Insects, rodents, & animals not present
- comments: no pest control logbook, current service report, and all other required documentation on-site. Must provide. Priority foundation violation 7-38-020(c)

41. Wiping cloths: properly used & stored
- comments: soiled wiping cloths were noted hanging on the inside/outside door of the 3 door prep cooler. Instructed to store in a container with a sanitizing solution between use.

45. Single-use/single-service articles: properly stored & used
- comments: all stored items in the rear dish area and the rear storage room must be elevated from the floors.

55. Physical facilities installed, maintained & clean
- comments: floors with dirt and debris next to the walk-in cooler, under and around the stored soiled linens. ----noted dirty ceiling light shield in the rear storage room. Must clean and maintain all.

- comments: no proof of food handler training or certificate for the employee on site. Must provide for all food handlers.
