- comments: found no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010. No citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: found procedure/plan on-site; however, kit not complete missing solution to control norovirus for responding to vomiting and diarrheal events, also missing mop etc. Instructed facility to maintain any appropriate supplies on site. Priority foundation violation 7-38-005.

- comments: must provide proper labels for date marking the preparation and consumption by/discard date (maximum of 7 days total) of all refrigerated, ready-to-eat, tcs foods prepared on-site and held over 24hrs. Priority foundation violation#: 7-38-005. No citation was issued.

- comments: rusty shelves in the storage area. Broken with holes exterior panel of two-door reach-in cooler(used for dry storage) prep table base is rusty unable to clean. Surface must be smooth and easily cleanable. Remove dirty cardboards on shelves under prep table

Must clean and maintain walls. #55)6-501. 114 observed garage area with excess clutter creating harborage for insects and rodents. Must remove all unnecessary items and organize all remaining items away from walls and up off the ground. #55)6-201. 11 observed foam sealant used to close holes in floor along south side of dining room. Must remove foam sealant and restore surface to a smooth and easily cleanable surface. Priority foundation violation:7-42-090, no citation issued
