2. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation manager certificate posted in facility. No city of chicago food sanitation manager on site while tcs foods (coffee drinks with milk, freshed squeezed juices, etc. ) are being prepped. Must have a valid city of chicago food sanitation certificate posted in plain view of customers at all times. Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Priority foundation violation 7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy on site. Must provide written employee health policy for all employees. Priority foundation violation. 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed incomplete vomit and diarrhea clean up kit on site. Must provide written procedure for the clean up of vomit and diarrheal events, and all corresponding equipment outline in procedure, including a sanitizer effective against norovirus. Priority foundation violation 7-38-005.
36. Thermometers provided & accurate
- comments: observed no thermometers inside coolers in front prep area. Must place a thermometer inside all refrigeration, in a conspicuous spot.
61. Summary report displayed and visible to the public
- comments: observed summary report blocked by bags stacked on the counter. Instructed must have summary report posted in plain view of customers at all times. Priority foundation violation 7-42-090.
