10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot running water at employee toilet room handwashing sink. Also observed no hot runing water at the handwashing sink upon arrival in the food prep area. Manager has to turn hot water on and off manually at the water master shut off valve. Temperature of water at the employee toilet room was 65. 9f. Instructed to have hot (100f) and cold running at all handwashing sinks. Also, must repair said handwashing sink in the food prep area, so the hot water can remain operable at all times. Priority violation 7-38-030(c), citation issued

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity. Observed two live roaches on the floor in the dishwashing area and on the floor near the food cart stored in the cold holding units' storage area. Also observed approximately 5-6 small flies in the corner of the three door cold holding unit in the food prep area, near the mop sink in the dishwashing/ food prep area. Also, observed approximately 6 mice droppings on the white pipe encase behind the two door refrigerator in the food prep area and behind the garbage in the office area. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: observed dead roaches on the floors near the cold holding units' storage area. Instructed to remove dead bugs and maintain.

39. Contamination prevented during food preparation, storage & display
- comments: observed a dry spice not covered while not in use on the food cart in the cold holding units storage area. Instructed to cover while not in use and maintain.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed the facility missing one sink stopper. Instructed to provide and maintain.

49. Non-food/food contact surfaces clean
- comments: observed dried brown residue in the two door cooler in the cold holding unit storage area. Also, observed on the interior of the stand-alone freezer at the botton animal blood. Instructed to clean said equipment and maintain.

- comments: observed the food cart stored in the cold hold unit storage area with dried up food residue and not maintained. Instructed to clean said cart and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed the floors under the hot holding unit under the service counter window in the cold holding unit storage area, behind the two door cooler in the food prep area, under the shelving units in the dry storage closet with a build up of debris and food. Instructed to clean and maintain said floors.

- comments: observed clutter on the side of the stand-alone freezer in the cold holding storage area. Instructed to elevate items, declutter and organize said corner and maintain.

60. Previous core violation corrected
- comments: previous core violations not corrected from 07/19/2021, report #2522899 #47 observed badly stained cutting boards in food prep area. Instructed to replace and maintain, clean and sanitize every two hours until replacement is made to avoid any possible contamination off foods. #55 observed a build up of dust, dirt and food debris on walls and floors under and around all cooking equipment and throughout food service area. Instructed to clean, sanitize and maintain. #56 observed no lighting under hood ventilation unit, instructed to provide proper lighting with protective light covering. #56 observed no protective light covering over lighting by stove and hood ventilation unit and no covering for lighting near the three compartment sink. Instructed to provide and maintain for food safety and sanitation. #57 observed two new employees without proper food handler training certifications. Instructed to obtain and provide on site at all times. #58 observed no allergen training onsite for city of chicago certified food service manager. Instructed to obtain and provide on site. Priority foundation:7-42-090, citation issued
