- comments: observed no person in charge with a city of chicago certificate. Instructed to have a certified food manager with the city of chicago certificate on site at all times.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (chicken etc. ) is prepared and served to customers. Instructed a city of chicago certified food manager must be on site at all times foods while (tcs foods) are being handled prepared and served. Priority foundation violation 7-38-012,citation issued.
49. Non-food/food contact surfaces clean
- comments: instructed to detail clean the sides of the deep fryers with grease build-up,interior of the microwaves and knozzles at the fountain drink station and maintain.

- comments: observed 2 ceiling tiles missing in front of the kitchen exhaust hood. Instructed to provide and maintain.

- comments: instructed to detail clean and maintain floors with grease build-up and dried food debris accumulation along the walls and in all corners throughout prep/cooking area especially under and around cooking deep fryer equipments including the floor drains also, detail clean walls in chicken prep/cooking area to remove accumulation of dust build-up and maintain.

- comments: instructed to replace burnt-out light bulbs in washrooms and maintain adequate lighting at all times.
