- comments: observed ventiltion covers in both toilet rooms with accumlated dust. Instructed to clean and maintain.

- comments: observed incomplete allergen awarness training. Instructed person in charge to ensure that anyone who has a city of chicago food sanitation certificate has completed allergen awarness training and maintain.
