- comments: no employee health policies on site for food and conditional employees. Instructed to provide and maintain. Priority foundation violation. 7-38-010. Citation issued.

- comments: observed rangoon stored directly on soiled rack inside refrigerator in food prep area. Food discarded (5lb, $35) all foods must be properly stored in food grade containers or bags and covered inside refrigeration/freezer units. Priority. 7-38-005. Citation issued.
37. Food properly labeled; original container
- comments: observed all bulk ( rice, flour, sugar,) and working food ingredients (sugar, salt, msg, spices) containers without labels. Instructed to provide labels with contents name.
38. Insects, rodents, & animals not present
- comments: observed large hole in rear screen door and 1/4' gap underneath door. Must repair door to prevent pest entry.
39. Contamination prevented during food preparation, storage & display
- comments: must store large plastic bins and carboard boxes of food off of floor in walk in cooler and freezer and store on shelving units.

- comments: observed food debris inside ice machine at smoothie station. Must clean side panels of ice machine.

- comments: observed no food allergen training for food manager. Must provide and maintain.
