- comments: observed no city of chicago certified foodservice manager on premises. Instructed city of chicago certified foodservice manager shall be on premises at all times. Priority foundation violation 7-38-012 citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy on site. Instructed to provide written employee health policy for all employees. Priority foundation violation 7-38-010 citation issued.

- comments: observed over 130 mouse droppings in facility, in areas such as space in between wall where grease trap is located, behind mixer and oven, prep table and shelf and in corner and along walls in room with hot water tank. Instructed to eliminate pest activity, remove mouse droppings and clean affected areas. Recommended pest control service facility on or after 11/21/2023. Priority foundation violation. 7-38-020(a) citation issued.

- comments: observed no sanitizing solution test kit on premises. Instructed to provide sanitizing solution test kit and maintain on premises at all times. Priority foundation violation 7-38-005 citation issued.

- comments: observed no self closing device on washroom door. Instructed to install self closing device on washroom door and maintain.

- comments: observed previous core violations from inspection report #2370127 dated 05/05/2020 not corrected. Instructed to correct. Priority foundation violation 7-42-090. Citation issued. Observed bulk food containers (sugar, flour, etc. ) without required labels. Instructed must label all bulk food containers with common name observed raw wood surfaces at front counter. Instructed must paint or stain these porous surfaces to make smooth and easily cleanable. Must maintain same. Observed leak at base of faucet of 3 compartment sink. Must repair and maintain same.
