- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and maintain at all times.

- comments: observed facility not meeting compliance with variance/specialized process; facility in (rop) reduce oxygen packaging of raw marinated beef, chicken, fish. Salsas etc in walk-in cooler and walk-in freezer in basement prep. Informed person in charge of requirements and facility must meet and maintain. Tag rop machine and manager immediately and voluntarily disposed of 80 lbs of products worth $2050. 00 through denaturing process. Priority violation 7-38-005 citation issued.

- comments: noted a vacuum packaging machine on site and assorted food items inside the walk in cooler vacuum packaged with no proof of haccp or variance per health code requirement. Instructed management discontinue the vacuum packaging process until a variance and haccp are approved by the health department. Priority foundation violation #7-38-005. Citation issued.

- comments: observed pest activity with approx. 25 small flies noted during inspection. Small fly activity was noted near the basement dish machine, 3 comp sink, and 1st floor bar. Instructed to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

- comments: observed rusted storage racks in walk in cooler, surfaces must be smooth and easily cleanable. Instructed to maintain

- comments: observed grease buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to degrease, clean and sanitize.

Observed dust buildup on fan in walk-in cooler, light shields in cooking area and ceiling and wall vents in prep and storage areas. Instructed manager to clean and maintain all fans, light shields and ceiling/wall vents.

- comments: observed chipped and bubbling water damaged paint on the ceiling of basement prep area. Instructed to resurface and repaint chipped areas to ensure walls are sealed, smooth and easily cleanable.
