Instructed all original/valid city of chicago food service sanitation manager's certificates must be posted for the public's view at all times. Priority foundation. Citation issued 7-38-012.
38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed a window opened, located in the customer dining area/food prep area, without adequate protection to prevent the entry of live pests and - or live insects. Observed the side door with a 1/2 inch gap around the door top. Instructed to maintain window closed or provide adequate protection such as a screen and make the side entrance door tight fitting and pest proofed.

- comments: 4-502. 13 observed some plates and bowls not inverted by the bar counter/customer serving area. Instructed to invert all tableware to protect from possible contamination.

- comments: 4-603. 15 observed no written procedure for the washing, rinsing, and sanitizing clean-in-place equipment (margarita machine). Instructed to provide a written procedure.

- comments: 5-205. 15 observed the 3-compartment sink, located by the 1st floor food prep area, continuously running and unable to shut off properly. Instructed to repair.

- comments: 6-201. 13 observed no coving on the wall in various areas of the basement food prep area, such as by the 3-compartment sink and along the walls under all dry storage shelves. Instructed to provide adequate wall coving.
