- comments: 4-301. 11 noted several cooling units are not working, including the in cooler (tag remains), only storing non-tcs foods stored inside at this time. Previously documented on initial report.

- comments: 4-204. 112(b) noted missing thermometers inside all cooling units on the premises. Instructed to provide and maintain.

They are as follows: 36) no metal stem thermometer on site for testing internal food temperatures. Priority foundation violation. 38)observed evidence of pest activity on the premise during the time of inspection. Noted approximately 30 live large black flies flying throughout butcher prep area, in rear prep area on prep table surfaces, and throughout rear prep area on freezer unit and stored dried kitchenware. Noted approximately 100 small rodent droppings scattered on the flothe walk-in-out walk-in cooler, shelving units in rear prep, and behind equipment in said space. Instructed to contact pest control for service, clean, and sanitize all said areas. Must eliminate activity. 48)no chemical test strips on site during the time of the inspection. Instructed to correct and maintain. Priority violation. #7-42-090. Citation issued.
