- comments: observed no consumer advisory disclosure on the menu for raw or undercooked foods. Management instructed to disclose or describe which foods can be ordered raw or undercooked and link them to the reminder statement on the menu. Must add the following sentence before the reminder statement:'these foods can be ordered raw or undercooked or may contain raw or undercooked ingredeients. ' priority foundation violation 7-38-005.

- comments: observed approx. 100 mouse droppings on the floor of the basement and on the storage tables in the basement. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: clean and remove the dead insects from the basement floor.

- comments: clean the prep tables below the grill and next to the 3 compartment sink, microwaves, 2 door prep cooler, tops of all chest freezers in the basement, interior of 2 door freezer next to the 3 compartment sink, and stove top.

- comments: must install a working self-closing door device on the men's washroom door.

- comments: clean the grease on the wall behind the cooking equipment. Clean the food debris on the basement floor along walls and under the storage tables and shelving units.

- comments: observed chipped paint on the wall next to the 2 door freezer above the mop sink. Observed unpainted drywall on the ceiling above the food prep area. Management instructed to scrape and repaint chipped paint on walls and unpainted drywall patch on the ceiling.

- comments: observed clutter behind the unused bar area and in the basement. Management instructed to clean and organize these areas to eliminate pest harborage conditions. Observed an extreme amount of clutter and unused equipment stored outside of the restaurant. Management instructed to remove clutter from this area to reduce pest harborage conditions.

- comments: clean the buildup of grease on the ventilation hood and filters above the cooking equipment.

- comments: previous core violation from report #2566823 on 11/9/22 not corrected: 55 - observed broken floor tiles in front of three compartment sink and in front of freezer located in the rear preparation area. Instructed the person in charge to repair and maintain in good repair. Priority foundation violation 7-42-090. Citation issued.
