- comments: 6-202. 15 exterior of the bottom front entrance door allows for pests. Exterior doors shall be self-closing, solid and tight fitting, and limited to designated use.
45. Single-use/single-service articles: properly stored & used
- comments: 4-903. 11(a;c)-- all stored items throughout in all storage hallways, rooms and closets must be elevated from the floors.
51. Plumbing installed; proper backflow devices
- comments: 5-205. 15-- broken cold water handle on the bar hand washing sink. ----leak under the dish machine room hand washing sink into a bucket. Must replace/repair all.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 11: noted a large hole on wall behind the chest freezer by the kitchen stove. Instructed to repair and maintain, seal all holes and openings where ever necessary to prevent hiding places for pest.
57. All food employees have food handler training
- comments: 2-102. 13--- no proof of food handler training or certificates for all employees. Must provide.
