- comments: 6-202. 13 observed rodent traps being set in the basement by the facility. Instructed to remove and only allow pest control to set all pest traps.

- comments: 3-305. 11 observed dry storage boxes being stored under a sewer line in the basement. Instructed to remove and do not store any items under sewer lines.

- comments: 4-601. 11(c) observed the interior of the ice machine with minor rust spots on the sides. Instructed to remove rust.
