- comments: pic doesn't have a certified food managers certificate must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (oatmeal,milk,bananas)must be on site at all times. Certified food manager arrived on site at 9:37 a. M. (priority foundation 7-38-012)(cos)(citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: must repair or replace slow drain at hand sink in explorers.

38. Insects, rodents, & animals not present
- comments: observed evidence of 3- live roaches on wheel bases of stove and 1- crawling on floor near 2- compartment sink floor drain. Also evidence of appx. 75 or more mice droppings inside of both fryer cabinets and on floor in between articles on floor in hot water tank room. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 6/14/24. (priority foundation 7-38-020(a)(citation issued)

- comments: observed several door not tight fitting along bottom and in center must make doors tight fitting to prevent rodent entry.

- comments: must provide sink stoppers for 3- compartment sink.

- comments: must clean dust build up from explorers -cubbies & shelves. Discovery-cubbies. Inquisitive-cubbies.

- comments: observed outside garbage dumpster lids in poor repair observed one lid broken and the center lid pushed down inside of said dumpster must make lids tight fitting. (priority foundation 7-38-020(b)(citation issued)

- comments: must scrape chipping paint on walls in explorers,discovery,creative and inquisitive class rooms.
