- comments: observed over 100 mice droppings scattered on floor along walls and corners in prep area, rear storage area, inside unused walk-in cooler, inside fryer cabinets, on floor and shelving units inside basement multi-use equipment storage closet at bottom of stairs, on shelving units and floors along walls and corners in basement. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed corn husks stored on floor in basement. Instructed manaager to elevate all food items six inches off floor.

- comments: observed handle of ice scoop laying on ice inside reach-in cooler. Instructed manager to store handle above ice to prevent contamination.

- comments: observed single service articles not inverted and stored on floor in basement. Instructed manager to invert and elevate all single service articles to prevent contamination.

- comments: observed food spills on walls in rear storage area, debris on floors under all equipment, along walls and in all corners in prep, storage and basement areas. Instructed manager to clean and maintain.
