- comments: observed tcs foods with evidence of spoilage in the walk-in cooler. The person in charge voluntarily discarded 20 lbs of ham valued at $60. 00 through denaturing process. Priority violation #7-38-005 citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed excessive black and pink slimy substance buildup on inside interior panel of ice machine and dripping onto ice for human consumption. Instructed the person in charge to remove buildup and detail clean the interior of the ice machine and maintain. Priority foundation 7-38-005. Citation issued

23. Proper date marking and disposition
- comments: observed tcs foods (chopped kale and ham ) exceeding the date of disposal. The person in charge voluntarily discard 30lbs valued at $70. 00 through denaturing process. Informed the person in charge that all prepared food items stored on site longer than 24 hours that shall be consumed on the premises, sold, or discarded can be held on the premises for a maximum of 7 days, with the day of preparation counted as day 1. Priority violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 30 mouse droppings on the floor behind bulk food containers in the basement preparation area, underneath the utensil storage rack in the rear wearwashing area, and underneath oven in rear prep area. Instructed the person in charge to remove all rodent droppings, clean surfaces, and maintain premises free of pests. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed multiple food handling employees not wearing hair restraints while preparing food. Instructed the person in charge to ensure all food-handling employees wear hair restraints while preparing food and maintain.

- comments: observed scoops for dry storage containers being stored directly inside of the container. Instructed the person in charge to store all utensils between use and maintain properly

- comments: observed food debris on floors throughout the millroom and basement prep area. ---also observed dirty floors drains in the rear warewashing area. Instructed the person in charge to detail clean floors and floor drains and maintain.
